RD-3 is a form used to submit a request for settlement of a Social Insurance Institution (ZUS) contribution payer account. It is a document submitted to ZUS. Its purpose is to settle the account of a contribution payer, i.e., an entrepreneur, a person conducting business activity, or another entity that pays social security and/or health insurance contributions.
