Based on the Art. 7 paragraph 1 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter GDPR, with submitting the form I am expressing my consent to have my personal data processed by the service provider for the purpose of the ordered service.
The following data is subject to data processing by the service provider:

- Surname,
- First name
- Date of birth
- Tax identification number
- Personal identification number (PESEL)
- Data on financial income (Income statements, invoices, bills)
- Financial data (bank account number)
- Addithonal data and documents sent via email message, except of the form contents.
