The ZUS PEL form is a document used in the Polish social insurance system to grant power of attorney to act on behalf of a contribution payer, an insured person, a benefit recipient, or a bailiff — by electronic means of communication or in person at ZUS (Social Insurance Institution) branches. Through it, the payer’s authorised representative can: register an entrepreneur, employee, or other insured person for social and/or health insurance; submit monthly contribution declarations; receive correspondence from ZUS.
